Positive List System in Korea: Issues and Challenges

Abstract
Objectives: The aim of this study is to describe the development of positive list system in Korea.

Methods: The study depends on the reviews on legal documents and selected literatures, and the personal experiences of an author.

Results: The Korean positive list system has been improved in terms of transparency, scientific rigor, and consistency since its implementation, while there has been concerns about the deteriorating access to new drug. Generally, the acceptance rate of HIRA is not different form those of other international agencies. The acceptance rate of anti-cancer drugs into the list is lower than the other categories of drugs, but it is comparable to other countries¡¯. MOHW announced the introduction of risk sharing scheme to improve the coverage level for serious diseases, and tried to find compromise between the efficiency and other social values.

Conclusion: To enhance the legitimacy and acceptability of resource allocation decisions, the public¡¯s opinions and preference needs to be reflected in the process.
